Same form. One fix.

Left is how most practice appointment forms behave today. Right is the same form with one small script added. Nothing else about the site changes.

Fill both forms, then press Request appointment on each and compare.

Your appointment form today

No checks. No confirmation.

Optional — leave blank if you don't have insurance.

The same form, fixed

Catches mistakes. Confirms it arrived.

Optional — leave blank if you don't have insurance.

Nothing typed on this page is sent, stored or seen by anyone.

What just happened

  1. The left form accepted a phone number with a digit missing. Nobody types a fake number on purpose — they drop a digit. The practice calls, the line doesn't connect, and the enquiry quietly becomes nothing.
  2. It accepted an email address that doesn't exist — gmial.com instead of gmail.com. Any reply bounces. The patient assumes she was ignored.
  3. It accepted an appointment date that has already passed. Usually the wrong year picked from the calendar. Someone at the front desk has to notice and call to ask what she actually meant.
  4. And then it said nothing at all. This is the one that costs bookings. She fills everything in, presses the button, and the page gives no answer. Some forms empty themselves, others just sit there. Either way she can't tell whether it worked, so she assumes it didn't and calls somewhere else. The practice never learns it nearly had her.
